The importance of textbooks in the shaping of world-language instruction in the US is undeniable: for better or for worse, published materials tend to define the content, pace, and pedagogical orientation of language courses at all levels in this country. As educators continue to confront the challenge of selecting and/or adapting published materials for their language courses (particularly for Spanish, which represents 69% of the total language textbook market at the high school level, and 52% at the college level), the need for information on the origin and evolution of our teaching materials becomes apparent. Existing medical Spanish curricula have improved language skills, but are incompatible with resident-physician schedules, and do not always integrate cultural education. A 2009 survey at our institution revealed that residents saw Spanish-speaking patients regularly and wanted a medical Spanish curriculum designed for them. Our objective was to improve medical Spanish and cultural competency among resident physicians at our institution. This article argues for translation as a fifth skill in language learning and for its integration in a systematic, purposeful, and informed manner in the Spanish language curriculum, with special attention to the situation in the United States. After reviewing the relevant literature in the fields of translation studies (TS) and language acquisition and teaching, we contend that a narrow, specialized, and one-dimensional view of translation, on the translation side, and a rigid ban on the use of the native language on the part of the SLA/teaching community prevented a fruitful integration of translation in the second language (L2) classroom. Research has shown that language barriers can impede access to healthcare and impact healthcare outcomes. Traditional Spanish courses have been criticized for not effectively addressing learners' specific needs, while Language for Specific Purposes (LSP) courses on healthcare frequently lack a credible methodological foundation. This paper examines the impact of a curriculum designed to develop students' Critical Language Awareness (CLA) in two postsecondary Spanish advanced grammar courses and a Hispanic Linguistics Master's course in a public institution of the US Northeast. The study includes a unique population of Latinxs, most of them born and raised outside the US, in the Dominican Republic. Quantitative analyses reveal CLA development in the areas of language variation, linguistic diversity, language ideologies, Spanish in the US, bilingualism, and translanguaging.
